When hyponatremia occurs, the resulting decrease in plasma osmolality (with the exception of the rare cases of non-hypoosmotic hyponatremia) causes water movement into the brain in response to the osmotic gradient, thus causing cerebral edema. A case of a 65 year old female with Altered sensorium

A case of a 65 yr old female with altered sensorium. My patient is a 65 year old  female housewife hailing from a rural district in south india. She presented to us with chief complaints of : Patient was brought to casuality on 23/11/22 at 4:00 am early  in the morning with complaints of loose stools since 5 days and pain as mentioned by the atttender as she was screaming and restless with altered sensorium. History of presenting illness : The patient was apparently asymptomatic 5 days ago. She passed loose stools- watery in consistency, foul smelling and blood tinged on consuming kaya churnam (an ayurvedic powder) , went to RMP nearby and was given medication,  even upon which loose stools didnot stop . The attender mentions she became weak and then on 23rd nov, apparently after eating curd rice ,  when patient slept , suddenly in the middle of night patient was screaming due to pain  and could not talk anything, as mentioned by the attender. She was then bought to casuality. History

CHEIF COMPLANTS C/O Fever (6days) C/O Arthralgia (4 days) C/O Headache (4 days) C/O Nausea (9 days) C/O Loose stools(5 episodes) HISTORY OF PRESENT ILLNESS  A 45 year male,daily worker by occupation came to casualty with complaints of high grade fever and chills and rigors relieved by taking medication associated with headache and goes off with fever.No retro orbital pain.H/O Arthralgia since 4 days. No morning stiffness.H/O generalized weakness for 3 days.patinet sought for consultation on 2 nd day of illnesse

CASE: 17 year old female complaints of vomitings and loose stools since 1week CHIEF COMPLAINTS: Vomitings since 1week Loose stools since 1week Fever since 3 days HISTORY OF PRESENT ILLNESS: Patient was apparently asymptomatic One week back,she had outside food followed by which she developed loose stools and vomitings. Loose stools:- 4-5episodes, water in consistency, non foul smelling, no blood tinged.
